Congratulations for participating in North American Jaguar Competition this year. According to the posted final results, you and your Jaguar are eligible for a fine trophy.

In order to receive the trophy, you need to fill out this form and, once it is completed, click on the Submit button to electronically send me the form. I plan to mail out all trophies to the recipients in plenty of time to enable you to pack the trophy in your luggage and bring it with you to the AGM if you wish to have it presented to you at that time. Therefore the sooner you return this form the better.
